SmartShopper

Maintaining a healthy lifestyle is driven by physical, psychological, social and environmental factors. We recognize this interplay and want to play an active role in the wellness of our employees by providing you and your loved ones with a new service to help your become better informed healthcare consumers. AvMed’s SmartShopper is an incentive and engagement program that is intended to help guide you to the best, most affordable medical options. The program’s goal is to educate you about the price differentials for common healthcare services in your area, then provide you with the tools to quickly and easily shop services based on price, quality, and location. If you choose a cost-effective location for your care, you can earn cash rewards. Currently, the program is available for Jackson Select HMO, Standard HMO, and POS plans.

The way SmartShopper works is:

  1. First, your primary care doctor must recommend or prescribe one of 36 common medical services, such as a MRI, mammogram, or colonoscopy.
  2. Once you have your prescription, you then have to call SmartShopper at 1-866-285-7453 or visit their website to find the low-cost, high-quality options in your area.
  3. Next, you choose the location for your procedure from one of the recommended options and make an appointment. Jackson facilities will always be the first tier choice with the highest reward. After you receive the prescribed service and your claim is paid, SmartShopper will verify that the location qualifies for an incentive, and you will receive a reward check in the mail ranging from $25 to $500, depending on the procedure. Reward distributions take about 45 to 60 days.

Change in Status

Changing your Benefits during the Plan Year
Within 30 days of a qualifying event, you must submit a Change in Status (CIS)/Election Form and supporting documentation (must be original or government certified) via fax to 305-355-2324. Upon the approval of your election change request, your existing benefit elections will be stopped or modified (as appropriate). However, if your election change request is denied, you will have 30 days, from the date you receive the denial, to file an appeal. Once you have faxed your forms, please call the FBMC Service Center at 305-585-6512 to ensure that it was received and approved.

Employee Assistance Program

Our Employee Assistance Program, Resources for Living, is designed for anyone who needs a little help managing life events, big or small. You and anyone in your household up to the age of 26 can call the confidential hotline at 786-466-8377 24 hours a day, and seven days a week. Services include but are not limited to:

  • Counseling services
  • Work-life balance services
  • Legal/financial services
  • Identify theft fraud resolution consultation
  • Web resources

Counseling services are available face-to-face, by phone, or video conference. You and anyone living in your household can receive up to three counseling sessions per issue, per person, per year with a licensed network professional at no cost to you. You do not have to worry about copays or deductibles. Resources are available to help assist with:

  • Work/life balance
  • Family and relationship
  • Depression
  • Conflict management
  • Alcohol/substance abuse
  • Stress management

Paid Time Off & FMLA

Employees eligible to take PTO accrue hours for every pay period worked. Hours are accrued in two separate banks- Personal Leave and Extended Illness. These banks are used for any time taken off, such as Vacation, Personal Time, Holidays and Sick time. Employees who have been employed with Jackson Health System for at least 12 months AND have worked at least 1,250 hours (actual hours worked) during the 12-month period immediately preceding start of requested FMLA leave, are also eligible to go out an FMLA leave.

Flexible Spending Account

A Flexible Spending Account (FSA) Lets you pay for eligible expenses, with tax- free money. You contribute to an FSA with tax- free money from your paycheck. This, in turn, may help you lower your taxable income. There are two types of Flexible Spending Accounts

Health Care FSA
A Health Care FSA is used to pay for eligible medical expenses which are not covered by your insurance or other plan. These expenses can be incurred by you, your spouse, a qualifying child or relative. Your full annual contribution amount is available at the beginning of the plan year, so you don’t have to wait for the money to accumulate.

Dependent Care FSA
The Dependent Care FSA is a great way to pay for eligible dependent care expenses such as before and after school care, day time baby-sitting fees, elder care services, nursery and preschool. Eligible dependents include your qualifying child up to age 13, spouse and/or relative.
You can request reimbursement from your Dependent Care FSA after your dependent receives day care services. Unlike the Health Care FSA, your full annual contribution is not available at the beginning of the plan year. You can only get reimbursed up to the amount that is available in your account at that time.

Life Insurance

Group Term Basic Life and Accidental Death and Dismemberment Insurance
Jackson Health System provides eligible employees with Group Term Basic Life Insurance in the amount of one times the employee’s annual base salary. In addition, Jackson Health System provides Group Accidental Death and Dismemberment Insurance (AD&D) with a value equivalent to the employee’s annual base salary in the event of death resulting from accidental injuries sustained whether on or off duty. Dismemberment benefits are payable for loss of hand, foot or sight of eye resulting from an accident. See your handbook for further details and plan limits.
Premiums for the Group Term Basic Life and AD&D coverages are paid in full by Jackson Health System.

Group Term Optional Life Insurance
Jackson Health System also offers additional life insurance, called Optional Life, at the employee’s expense. You may elect to purchase between one and five times your annual base salary for a maximum coverage of $2 million. You may obtain up to three times your basic annual salary without being subject to medical approval during your initial eligibility period.
To update your beneficiaries for Basic/Optional Life Insurance, please log on to Lawson Employee Self Service and click on the beneficiaries link under the benefits tab on the left hand side of the screen.

Short and Long Term Disability

How long is the benefit period? Up to 24 weeks for one period of disability. To age 65 or later, depending on the age at which you were disabled.
How long must I be disabled before I receive benefits? The latter of: Use of all extended illness/accumulated sick leave, or 14 consecutive days. 180 consecutive days of Total Disability.
What is considered a disability? Unable to do the material duties of your job; and Under the regular care of a physician As a result of injury or sickness. For the first 24 months for which a
Monthly Benefit is payable, you cannot perform the substantial and material
duties of your Regular Occupation. After a Monthly Benefit has been paid for 24
months, you cannot perform the material duties of Any Occupation.
Must I pay my premiums after my disability claim is approved? Yes No premium is due for Insured while he/she is receiving Monthly Benefits.
What if I am partially disabled? (Residual Disability) Not applicable “Partially Disabled” and “Partial Disability” mean that as a result of an injury or sickness you are capable of performing the substantial and material duties of your Regular Occupation on a part-time basis or some of the substantial and material duties on a full-time basis. An Insured who is Partially Disabled will be considered Totally Disabled, except during the Elimination Period. “Residual Disability” means being Partially Disabled during the Elimination Period. Residual Disability will be considered Total Disability.
What happens to my coverage if I leave Jackson Health System employment? Not applicable If you terminate employment or if coverage ends for any reason (except non-payment of premium), you can convert this plan to an individual policy by applying for conversion within 31 days of termination of insurance. You do not have to submit evidence of good health if you are applying within 31 days. Contact Reliance Standard Customer Service at 800-351-7500 to obtain a conversion application.
What happens if I’m on a leave of absence and/or on an out of pay status? Coverage will terminate. However, if an employee is currently under a disability claim and Leave of Absence from Jackson Health System occurs, the employee is able to make payments for benefits through FBMC. Payments must be made for the duration of the claiming period. If an employee is currently under a disability claim and Leave of Absence from Jackson Health System occurs, the employee is able to make payments for benefits through FBMC. Payments must be paid until the claim has been approved.

Tuition Reimbursement

In keeping with our commitment to help our employees succeed, Jackson Health System offers tuition assistance for employees interested in advancing their careers. Employees may apply for courses leading to a degree or certificate. Courses taken in non-degree seeking status are evaluated on an individual basis. Only eligible, pre- approved courses are reimbursed after required documents are submitted.

Defined Benefit Retirement Plan

The PHT Defined Benefit Retirement Plan was created in 1996 and has been available to employees hired after January 1, 1996. Current employees vest after 6 years of service, and Normal Retirement age is 65, or age 62 if hired prior to April 2012.

Eligible Jackson employees will be able to access their retirement benefits online via the eePoint Employee Self-Service system. This system is a web-based tool that allows you to calculate estimated retirement funds and forecast various payment scenarios based on age and retirement date. You can also review current plan information, view your summary pension plan description online, and access contact information to learn more about your pension and other benefits. Smoking Cessation

If you are thinking about quitting smoking, Jackson Health System’s Tobacco Cessation Program can help!

  1. Who Can Participate?
    All JHS patients, visitors and employees are welcome to register for classes.
  2. How much does the program cost?
    The program is absolutely free to anyone who would like to participate.
  3. How long is the program?
    You can join our six week session that meets for one hour a week or sign up for a one-time 90-minute session.
  4. Are classes available in English and Spanish?
    Yes, English and Spanish classes are available weekly.
  5. Where are the classes held?
    Classes are held at clinics and facilities throughout JHS. Please call 305-585-5319 for details or to make an appointment
  6. Do you get anything for completing the program?
    You’ll get a certificate of completion at your final class, and an invitation to become a program advocate to spread the word about our program. Plus, you’ll have a support group ready to help you when you need it.
