Jefferson Reaves, Sr. Health Center is a focal point for healthcare services in Overtown and Miami-Dade communities. It is committed to the dignity, social well-being and health of the community and the patients who are served at the center. The health center is the home of the University of Miami run Family Medicine Residency Training Program. The center offers comprehensive primary care services and, through the participation in the Jackson Health System, facilitates referrals to Jackson Memorial Hospital for specialized services. At the Jefferson Reaves center, staff provides care and treatment in three different languages: English, Spanish and Creole.

If you do not have health insurance, your financial evaluation will determine what amount you have to pay. To assist you in qualifying for health benefit programs, please bring the following information:
- Photo ID – drivers license, passport, Florida ID
- Proof that you live in Miami-Dade County – rent receipt, light, water or other utility bill
- Proof of income and family size – last four consecutive pay stubs, most recent income tax, bank statements

From S.R. 836 East, I-395 East or I-95 South
Take the FL-836 East/I-395 East ramp to I-95 East.
Merge onto FL-836 East.
Take the I-95 South exit toward Downtown.
Keep right at the fork; follow signs for N.W. 8th Street/Port of Miami.
Turn right onto N.W. 8th Street.
Take the 2nd right onto N.W. 5th Avenue.
Jefferson Reaves, Sr. Health Center will be on the northeast corner of N.W. 5th Avenue and N.W. 10th Street.
You can access the Jefferson Reaves, Sr. Health Center from Miami-Dade transit bus number 77 and from the Metrorail via the Culmer (Overtown) Station.
