Admission & Registration
All inpatient admissions and outpatient registrations are part of Patient Access Services. Patient Access Services also makes appointments for clinics and routine laboratory and radiology tests. If you have questions about being admitted into the hospital or registering for any of our clinics, please call 305-585-5225.
To be admitted, you need to be referred by your primary physician.
When you call for an appointment, you will be advised with what to bring. In most cases, we would register you in advance so you can report directly to your appointment.
If you have insurance, please bring the following:
- Photo ID

Upon admission, you should receive a copy of our patient handbook. The handbook will include details about the services available at Jackson Memorial and the surrounding medical campus, as well as important information on how to access your medical records, your patient rights and responsibilities, and what to expect during your stay.
