Please Read Before Proceeding

You must complete the required EMR training class before filling out this EMR Access Form. If you have not yet completed the training, please visit the Jackson Health System Online Orientation (jhsmiami.org), select Cerner PowerChart Viewer Access as your Account Type, and complete all Cerner training modules.

Please note:

  • EMR access will be granted only after the study has been approved by the JHS Office of Research Administration.
  • Once your account becomes active, you will be able to access the system only while physically located at a JHS or UM facility.
