RHISE Program Launches at Holtz Children’s Hospital and The Women’s Hospital at Jackson Memorial
Miami, FL – Jackson Health System is launching the RHISE Program at Holtz Children’s Hospital and The Women’s Hospital at Jackson Memorial. RHISE stands for reducing hypertension in pregnant and postpartum women through interventions and supportive education.
This three-year program is funded by the Department of Health, and uses virtual technology and a community health worker model to support moms who are at risk of hypertensive and/or diabetes disorders, as well as those who have additional general health, mental health, and/or social needs. The goal is to reduce social determinants of health and promote health equity while addressing the social needs of mothers in our community.
RHISE will also be integrated into The Women’s Pavilion at Jackson North and The Women’s Center at Jackson South.

Daily patient engagement will occur through a cloud-based technology designed to recognize trends in hypertension, diabetes, and related disorders. A doula is available to support the mom during and post- pregnancy. Wrap-around support will also be provided through care coordination with health workers to address any non-clinical challenges related to social determinants of health
